What to Expect During
Your First Visit

This first office visit experience is unique in that it is tailored to the needs of each individual patient. Appointments are scheduled to allow Dr. Crook ample time to fully understand the symptoms and physical history of his patient. This first visit is generally 2 hours in length.

After a comprehensive evaluation, you will receive a treatment plan. This treatment plan may consist of all or some of the following: osteopathic medicine treatment (OMT), therapeutic exercise, dietary and lifestyle modifications, injections, referrals, or radiological studies.

Dr. Crook will discuss the nature of your condition and try to answer any questions that you may have.

When you arrive for your first visit, please remember to bring with you:

  • Prescriptions

  • Relevant imaging reports

  • Your completed patient documents that can be found on our web site under  “Forms”

  • Medical records pertaining to your condition

Please be on time for your appointment:  We schedule our patients so that they have less than a 10 minute waiting time. If you arrive late, we cannot guarantee your full allotted treatment time nor can we guarantee that you can still be seen that same day. Cancellation of appointments have to be done 2 days prior to visit or a $100 late fee will be applied.
